PCP Cohorts

Cohorts are the learning space where PCP Team Leaders develop their Relational Leadership™ practices, learn about patient and community engagement and health equity, and initiate local action to advance primary care. They are a set of academic-year collaboratives of three to five teams, where participants and their Faculty Advisors gain the essential skills, knowledge, experience, and strategic support necessary to become change makers. Led by a Mentor from PCP’s Leadership Pathway, Cohort participants learn and test new skills through coaching calls and case consultation along with a group of fellow teams from around the country.

In the Cohort program, teams may work to advance their own initiatives, like hosting case conferences on interprofessional teaming, advocating for the integration of leadership into their institutions’ curriculum, or creating student-run free clinics that serve the most vulnerable members of their communities.
